Zusammenfassung
Hintergrund
In dieser Arbeit sollen die Indikation, das Prinzip, die operative Technik und die Ergebnisse der Hämorrhoidalarterienligatur/Rectoanalrepair (HAL/RAR) dargestellt werden. Die Operation wird in einzelnen nachvollziehbaren Schritten demonstriert. Die HAL/RAR ist eine operative, nichtresezierende Methode zur Behandlung des prolabierenden Hämorrhoidalleidens. Neben der dopplergesteuerten Ligatur der in das Corpus cavernosum recti einstrahlenden Äste der Arteria rectalis superior zur Reduzierung des arteriellen Zuflusses (HAL) erfolgt eine zusätzliche Reposition des prolabierenden Gewebes mit einer fortlaufenden Mukopexienaht (RAR, „rectoanal repair“).
Material und Methoden
Neben einer kurzen Darstellung der Literaturergebnisse werden eigene Daten aus der Deutschen RAR-Studie vorgestellt: Es handelt sich dabei um eine prospektive Kohortenstudie an 498 Patienten aus 14 Kliniken und proktologischen Praxen, bei denen ein prolabierendes Hämorrhoidalleiden mit HAL/RAR versorgt wurde. Die Nachkontrolle erfolgte in festgelegten Intervallen über 3 Jahre mittels Befragung und proktologischer Untersuchung.
Ergebnisse
Bei fehlenden Majorkomplikationen und nur wenigen Minorkomplikationen (4,2 %) waren nach nur einer Behandlung postoperativ alle Symptome dauerhaft deutlich gebessert: keine oder nur geringe Blutung 97 %, kein Prolaps 91 %, keine Schmerzen 95 %, kein Puritus (83 %), kein Stuhlschmieren (90 %). Der Beattie-Score nahm von initial 5,52 auf 0,6 ab.
Diskussion
Die HAL/RAR ist für alle symptomatischen, prolabierenden Hämorrhoidealstadien geeignet. Sie ist befundadaptiert und individualisiert einsetzbar, hat wenige Komplikationen und sehr gute Langzeitergebnisse. Die Methode ist auch bei Problembefunden einsetzbar (Inkontinenz, Vorbestrahlungen, Voroperationen).
Abstract
Background
This article presents the indications, principles, operative technique and the results of hemorrhoidal artery ligation rectoanal repair (HAL-RAR). The procedure is described in a step-by-step fashion. The HAL-RAR is a surgical non-resecting procedure for the treatment of prolapsed hemorrhoids. It includes both the Doppler-guided ligation of the superior rectal artery branches that feed the hemorrhoidal plexus (corpus cavernosum recti) in order to reduce the arterial inflow (HAL) and mucopexy of the prolapsed tissue with a running suture (RAR).
Patients and methods
Apart from a brief discussion of the literature we present the outcomes of the German RAR study. This study is a prospective cohort study including 498 patients from 14 hospitals and proctological clinics, which performed HAL-RAR for prolapsed hemorrhoids. Follow-up was realized in predefined intervals for 3 years and included an interview and a proctological examination.
Results
There were no major complications and only few minor complications (4.2%). Patients indicated an improvement after the procedure for each of the following symptoms: no or only minor bleeding 97%, no prolapse 91%, no pain 95%, no pruritus 83%, no soiling 90%. The Beattie score dropped from initially 5.52 to 0.6.
Discussion
The HAL-RAR is an appropriate procedure for all symptomatic prolapsing stages of hemorrhoidal disease. It can be tailored to the extent of the hemorrhoids taking individual findings into account. Complications are rare and long-term results are convincing. It can also be performed in pre-existing pathologies, such as anal incontinence, previous radiation treatment and a history of anal operations.
