Konservative vs. operative Behandlung bei Oberarmschaftfrakturen

Zusammenfassung

Trotz der guten Möglichkeit einer konservativen Behandlung von diaphysären Humerusfrakturen sprechen bei den zur Verfügung stehenden operativen Verfahren der frühe Funktionsgewinn mit der Verhinderung von dauernden Beeinträchtigungen sowie der deutlich höhere Komfort für Patienten für die operative Behandlung. Mit der Verriegelungsmarknagelung steht ein gering invasives Verfahren mit niedrigerer Komplikationsrate zur Verfügung. In Abhängigkeit von Frakturform, Frakturlokalisation und dem Vorliegen von neurovaskulären Begleitverletzungen stellt aber die Plattenosteosynthese in der Hand des erfahrenen Chirurgen das geeignete Verfahren dar.

Abstract

Although conservative treatment of diaphyseal humeral fractures offers a good possibility of sound healing, the early functional gain with prevention of permanent impairment and the decidedly greater convenience to the patient support the implementation of one of the operative procedures available. Use of the locking nail is the method most frequently applied, because it is a minimally invasive method with a low complication rate. Depending on the form and location of the fracture and the presence or absence of neurovascular injuries, however, plate osteosynthesis is the method of choice for the experienced surgeon.
