Zusammenfassung

Die Lisfranc-Gelenklinie ist durch eine sehr feste, ligamentäre Verbindung der 5 Mittelfußknochen an die kuneiformen Fußwurzelknochen I–III und das Kuboid gekennzeichnet. 90% der Luxationen gehen mit Frakturen vorzugsweise der Mittelfußknochen einher. Die Röntgendiagnostik bedarf wegen der räumlichen und komplexen Skelettanatomie einer sorgfältigen Bildanalyse und wird im Verletzungsfall durch eine CT-Untersuchung ergänzt. Wesentliches Therapieprinzip ist die frühzeitige offene Reposition mit anatomischer Fixation der Gelenkpartner und Frakturen mittels Spickdrähten und Einzelschrauben.

Abstract

The Lisfranc articulation is characterized by a very firm ligamentary connection between the five metatarsal bones and the cuneiform bones corresponding to digits I–III and the cuboid. Dislocations are accompanied by fractures in 90% of cases, the metatarsal bones being affected particularly frequently. Because of the complex spatial anatomy of the skeleton, X-rays have to be carefully analysed and should be supplemented by CT scans if injuries are present. The most important object of treatment consists in early reduction with anatomical fixation of fractures and articulations by means of K-wires and single screws.
