Weichteildefekte, die keinen Primär- oder Sekundärverschluss erlauben, erfordern differenzierte Therapiekonzepte. In vielen Fällen bieten tangentiale Hautzugverfahren neben Gewebetransplantationen alternative Behandlungsmöglichkeiten. Durch intermittierenden Zug auf die Wundränder kann eine sukzessive Verkleinerung der Defektfläche erreicht werden. Hierbei werden die Prinzipien der Naht mit den Vorteilen der offenen Wundbehandlung kombiniert. Mit dieser Methode gelingt in > 70% der Fälle ein vollständiger Wundverschluss der sekundär heilenden Weichteildefekte über eine Kombination aus Dehnung und Wachstum der Haut. Tangentiale Hautzugverfahren bestechen durch eine große Indikationsbreite und Praktikabilität, geringen Kostenaufwand und gute kosmetische Ergebnisse.

Abstract

Soft tissue defects that are not suitable for primary or secondary wound closure demand individually tailored treatment strategies. In addition to tissue transplants, many patients benefit from skin traction techniques. The application of intermittent traction on the skin at the edges of the wound results in a steady decrease in the size of the defect. This combines the advantages of open wound treatment and the principles of skin suturing. With this method complete wound closure is achieved in over 70% of all cases of secondary wound healing, by way of a combination of skin stretching and growth of the skin. Skin traction techniques are attractive in that they are indicated in a wide variety of circumstances and are not difficult in practice, they are inexpensive, and they give good cosmetic outcomes.
