Patienten-Daten-Managementsysteme in der Intensivtherapie

Zusammenfassung

Ein PDMS (Patient Data Management System) ist ein für die Intensivtherapie spezialisiertes EDV-System mit einer Funktionalität, die Dokumentation während der Intensivtherapie im weitesten Sinne beinhaltet. PDMS werden vor allem zur präzisen Darstellung des medizinischen Verlaufes einerseits und zur Erfassung von Qualitäts- und Kostenfaktoren andererseits angewendet. Die Aufgaben lassen sich trennen in solche allgemeiner Art wie stationsbezogene Patientenverwaltung, Behandlungsplanung, Pflegeplanung, Qualitätssicherung, Kostenkalkulation und in Aufgaben, die die bettseitige Dokumentation beinhalten wie Behandlungsdokumentation (Verlaufskurve), Pflegedokumentation, Trenddokumentation.

Eine besonders wichtige Eigenschaft eines PDMS besteht in der Kommunikation des Systems mit externen Rechnern (Apotheke, Röntgen, Labor usw.), die in vielen kommerziellen Systemen zum heutigen Zeitpunkt noch unzureichend realisiert ist. In der Zukunft wird die Kommunikation nach “außen” einen noch größeren Stellenwert einnehmen, da man am Arbeitsplatz über das heutige Informationsangebot hinaus auch auf spezielle medizinische Datenbanken, Expertensysteme und Konsiliardienste zugreifen möchte.

Patient data management systems (PDMS) are hardware/software products especially designed for the documentation of treatment in intensive care medicine. The advantages of PDMS are continuous high quality documentation, comparability of data due to the availability of a data bank, additional information at the bedside, precise data for the calculation of the overall cost of the patient's stay. The technical design of PDMS depends on an integration of network technology into the hospital. It will be appropriate to conncect the systems to the Internet, but safety requirements have strongly to be followed. Concerning the database, client server architecture as well as language standards like SQL should be used. Object oriented databases will be available in the near future. Another future goal of PDMS will be using knowledge based technologies within these systems. Drug interactions, disease related diagnostic and/or therapeutic techniques and other information sources can be integrated.

At this time, almost none of the commercially available systems have matured to a point where their purchase can be recommended without reservation. There is still a lack of standards for the subsequent exchange of data, and a solution to a number of ergonomic problems still remains to be found. Nevertheless, PDMS will be required in the near future. The advantages of accurate documentation and quality control in the presence of careful planning outweigh cost considerations by far.
