Zusammenfassung
Anorganische und organische Chemikalien verursachen bei vielen Menschen bei wiederholtem Hautkontakt eine allergische Kontaktdermatitis, eine vom Immunsystem verursachte entzündliche Hauterkrankung. Mehr als 4.000 Kontaktallergene sind bekannt. Die Effektorzellen, die zur Hautschädigung führen, sind v. a. zytotoxische CD4+- und CD8+-T-Zellen. Diese T-Zellen werden in den hautdrainierenden Lymphknoten von dendritischen Zellen (DC) aktiviert. Die DC werden selbst im Zuge der kontaktallergeninduzierten Hautentzündung aktiviert, wandern aus der Haut aus und präsentieren Kontaktallergene auf MHC-Molekülen spezifischen T-Zellen. Chemikalien-induzierte Entzündung, für die ich den Begriff Xenoinflammation vorschlage, ist für die Sensibilisierung gegen Kontaktallergene essenziell. In den letzten Jahren konnte gezeigt werden, dass Kontaktallergene Entzündungsmechanismen aktivieren, die normalerweise der Infektabwehr dienen. Die Analogien zwischen Infektion und allergischer Kontaktdermatitis haben eine Reihe von Implikationen für die mögliche Beeinflussung der Kontaktallergie durch Pathogene und für die Entwicklung kausaler antientzündlicher Therapien.
Summary
Upon repeated skin contact inorganic and organic chemicals cause allergic contact dermatitis, an inflammatory skin disease caused by the immune system in many human subjects. More than 4.000 contact allergens are known. The effector cells that cause damage to the skin are mostly cytotoxic CD4+ and CD8+ T cells. These T cells become activated in skin-draining lymph nodes by dendritic cells (DC). The DC become activated themselves in the course of the contact allergen-induced skin inflammation, emigrate from the skin and present contact allergens on MHC molecules to specific T cells. Chemikalinduced inflammation, for which I propose the term xenoinflammation, is essential for the sensitization to contact allergens. In the last years it was demonstrated that contact allergens activate inflammatory mechanisms that normally serve the defence against infections. The analogies between infection and allergic contact dermatitis have a number of implications for the potential modulation of contact dermatitis by pathogens and for the development of causative anti-inflammatory therapies.
