A syndrome of iris nodules, ectopic descemet’s membrane, and unilateral glaucoma

Summary

Two cases are reported, and one case is cited from the literature, constituting a clinico-pathologic syndrome in which pedunculated nodules occur in a sector of the iris associated with focal ectropion uveae, ectopic Descemet’s membrane and glaucoma. All three patients were adult women and the stated history indicated recent origin of symptoms. The nodules were interpreted clinically as melanomas, but were more consistent pathologically with unusual nevi. Some of the findings suggested a congenital basis for the abnormalities while the overall appearance suggested a similarity to the nodules described with neurofibromatosis, but in the present patients there was no other evidence to support such an association.

Résumé

Rapport de deux cas auxquels on ajoute une observation trouvée dans la littérature, qui constituent un syndrôme anatomo-clinique caractérisé par la présence de nodules dans un secteur de l’iris, par un ectropion de l’uvée, une ectopie de la membrane de Descemet et un glaucome. Les trois malades étaient des femmes adultes et d’après les antécédents les lésions paraissaient récentes. Cliniquement les nodules iriens ont été considérés comme des mélanomes, mais du point de vue anatomo-pathologique le diagnostic de naevus atypique semble plus correct. Certains faits font penser à une origine congénitale de ces anomalies qui dans l’ensemble paraissent s’apparenter aux nodules décrits dans la neurofibromatose. Toutefois chez les malades en question aucun autre signe de maladie de Recklinghausen n’a pu être découvert.

Zusammenfassung

Es werden zwei Fälle beschrieben und es wird noch eine dritte Beobachtung aus der Literatur erwähnt. Es handelt sich hierbei urn ein anatomo-klinisches Syndrom, das durch Irisknötchen, ein Ektropion des Uvea, eine Ektopie der Descemetschen Membran and ein Glaukom gekennzeichnet ist. Die drei Kranken waren erwachsene Frauen und nach der Vorgeschichte waren die Krankheitszeichen erst kürzlich aufgetreten. Klinisch wurden die Iris-knötchen als Melanome angesehen, die anatomo-pathologische Diagnose eines atypischen Naevus scheint jedoch eher der Wirklichkeit zu entsprechen. Einzelne Anzeichen lassen an einen kongenitalen Ursprung der Anomalien denken. Diese stehen jedenfalls den in der Neurofibromatose beschriebenen Irisknötchen nahe, es bestand aber bei den beschriebenen Patientinnen kein anderer Anhaltspunkt für eine Recklinghausen’sche Krankheit.
