Coeliakie is een chronische darmziekte die geïnduceerd wordt door een overgevoeligheid voor de opslageiwitten van graan (gluten). Uit recent onderzoek in de bevolking blijkt dat coeliakie vaker voorkomt dan eerder werd gedacht. In een normpraktijk komen 12 patiënten met coeliakie voor. Bij slechts 1 op de 7 patiënten is deze diagnose gesteld. Niet alle patiënten hebben het klassieke beeld met chronische diarree, gewichtsverlies en buikpijn. Ook anemie, botontkalking en andere uitingen van deficiënties kunnen de eerste manifestaties zijn. Serologische tests hebben een hoge sensitiviteit en specificiteit; bij patiënten met klachten is de positief voorspellende waarde groot genoeg zodat slechts weinig patiënten ‘voor niets’ naar de maagleverdarmarts worden verwezen. Voor huisartsen is het zeker een optie om deze serologische tests aan te vragen als volgende stap in de diagnostiek bij een anamnese die doet denken aan coeliakie.
