Achtergrond Gegevens over de effectiviteit van de poortwachter spelen nauwelijks een rol in de discussies over de wenselijkheid van de poortwachtersfunctie. Wij deden een literatuuronderzoek naar het effect op passend zorggebruik.

Methoden Searches in verschillende elektronische databanken en in Huisarts en Wetenschap en Medisch Contact.

Resultaten Wij vonden 13 onderzoeken van wisselende kwaliteit, waarin sterk verschillende triagevormen werden bestudeerd. Triage bleek een aanzienlijke vermindering van onnodig eerstehulpbezoek te bewerkstelligen (bij kinderen tot 75% minder). Bij volwassenen met niet-acute hartaandoeningen of met luchtweginfecties was er geen duidelijk verschil in zorggebruik. De meeste onderzoeken lieten geen nadelige effecten zien in de zin van gemiste pathologie of ontoereikende zorg. Er zijn aanwijzingen dat patiënten met zeldzame aandoeningen beter af zijn met directe toegang tot specialisten.

Conclusie Empirisch onderzoek naar de effectiviteit van de poortwachter is schaars en van wisselende kwaliteit en komt grotendeels uit de VS. De poortwachter lijkt niet bij alle gezondheidsproblemen passende zorg te bevorderen. Verder prospectief onderzoek, bij voorkeur internationaal vergelijkend van aard, is nodig.

Abstract

Willems DL, Veldhuis M. Does the gatekeeper enhance appropriateness of care? A literature study. Huisarts Wet 2004;47(13):625-30.

Background Empirical data on the effectiveness of the gatekeeper hardly play a role in discussions about the desirability of gatekeeping in health care. We performed a literature search for data on the effects that gatekeeping might have on appropriateness of care.

Methods Searches were carried out in Pubmed (‘gatekeeping’, ‘gatekeeper’, ‘appropriate(ness)’ and ‘triage’) and in the registers of Huisarts & Wetenschap and Medisch Contact.

Results 13 studies of varying quality were found, 11 of them from the United States. There were considerable differences in the types of triage in these studies. Triage may lead to considerable reduction in unnecessary emergency visits, e.g. for children (differences of up to 75% have been reported). No clear effect was seen on health care use in patients with non-acute cardiac or respiratory complaints. One study reported a weakening of co-ordination of care in a gatekeeper managed care programme. Most studies showed no indications of hazardous effects of gatekeeping with regard to missed diagnoses or insufficient care. Retrospectively there were indications that patients with rare diseases would have been better off in a direct access system.

Discussion Empirical studies on the effectiveness of the gatekeeper are rare and of uneven quality. Gatekeepers do not seem to improve appropriateness in all health problems. Further prospective studies are needed, preferably international comparisons.
