Knobbel in de borstanamnese borstkanker borstklieraandoeningen laboratoriumonderzoek lichamelijk onderzoek mammografie

Samenvatting

In Nederland zal de huisarts bij een knobbel in de borst zelf het risico inschatten en het beleid daarop afstemmen. Veel vroeger geleerde symptomen en klachten hebben geen voorspellende waarde of zijn zo zeldzaam dat de betekenis voor de dagelijkse praktijk verwaarloosbaar is. Lactatie in de voorgeschiedenis, langer bestaande ingetrokken tepel, melkachtige afscheiding (uniof bilateraal), pijn of verschil in grootte hebben geen betekenis. Een ulcus of sinaasappelhuid wordt tegenwoordig niet meer gezien. Van veel groter belang zijn de leeftijd van patiënte, de (familiaire) voorgeschiedenis en eventuele genetische gegevens. Hoewel het aanvullend beeldvormend onderzoek binnen de diagnostiek niet meer is weg te denken, moet het vanwege de grote kans op foutpositieve uitslagen bij een lage voorafkans alleen op indicatie gebruikt worden.
