Tussen paternalisme en consumentisme Het dilemma van de huisarts

Samenvatting

Dit artikel beschrijft en vergelijkt de ideeën van huisartsen en patiënten over de huisartsenzorg in Nederland. Door middel van vragenlijsten werd patiënten en huisartsen gevraagd welke van 40 aspecten van huisartsgeneeskundige zorg ze belangrijk vonden en hoe ze oordeelden over 23 van die aspecten. Er bleek grote overeenkomst te bestaan tussen de verwachtingen en oordelen van huisartsen en patiënten. Opvallend is dat het voor patiënten net zo belangrijk was als voor huisartsen dat het nut van medicijnen, adviezen, medisch onderzoek en verwijzingen kritisch beoordeeld wordt. Huisartsen onderschrijven net als patiënten het belang van patiëntgericht werken. Daarnaast waren er ook verschillen. Patiënten legden meer nadruk op het belang van de beschikbaarheid van de huisarts en de intensiteit van de informatievoorziening en huisartsen beaccentueerden meer het belang van de organisatie van de huisartsenzorg. Huisartsen zijn daarnaast kritischer dan patiënten over de door hen geleverde zorg, met name wat de intensiteit van de informatievoorziening en de kwaliteit van de arts-patiëntrelatie betreft. Huisartsen denken bovendien dat patiënten nog kritischer zijn dan zijzelf.

De grote overeenkomst tussen de ideeën van huisartsen en patiënten maakt dat het vertrouwen gerechtvaardigd is dat huisarts en patiënten min of meer dezelfde opvattingen hebben over verlening van adequate, effectieve, efficiënte, maar ook patiëntgerichte zorg. Dialooggerichte huisartsenzorg slaat een brug tussen het paternalisme van de huisarts en het consumentisme van de patiënt waardoor er een evenwicht gevonden kan worden tussen evidence-based huisartsgeneeskundig werken en de wensen en verwachtingen van de patiënt.
