Samenvatting
In een dwarsdoorsnedeonderzoek bij 278 werkenden in het kader van een preventief medisch onderzoek (PMO) gericht op preventie van harten vaatziekten is een vergelijking gemaakt tussen de resultaten van een inspanningselektrocardiogram (XECG), SCORE-bepaling volgens de richtlijn Cardiovasculair risicomanagement en een Carotid Intima Media Thickness + Plaque echografie (C-IMT+P) van de halsslagaders. C-IMT+P stelt echografisch alle stadia van atherosclerose vast.
Uit de analyse van de onderzoeksresultaten blijkt dat er een significante correlatie volgens Pearson bestaat tussen de SCORE-bepaling en de gemiddelde vaatwanddikte uit de C-IMT+P. Deze correlatie ontbreekt bij de vergelijking met de resultaten uit het XECG.
De keuze van onderzoeksmethoden bij PMO dient gebaseerd te zijn op reproduceerbaarheid, validiteit en veiligheid van de onderzoeksmethode. Op basis van onze onderzoeksresultaten is de toepassing van C-IMT+P bij PMO naar hart- en vaatziekten van werkenden te prefereren boven de toepassing van XECG. Daarnaast heeft C-IMT+P-onderzoek een positieve invloed op therapietrouw en motivatie voor leefstijlverandering.
