Abstract Summary The treatment of demented elderly with antipsychotic drugs
In the Netherlands nearly one third of the nursing home residents uses antipsychotic drugs, often for a long time. Research data of the adverse effects of the majority of antipsychotics in demented elderly is scarce.The positive effects on behaviour are limited compared with placebo. Nearly all recent studies show a higher mortality within 3 months. Conventional antipsychotics are presumed to be as harmful as atypical antipsychotics. It is important to be more reluctant to prescribe antipsychotics in dementia.

Samenvatting
In Nederland krijgt bijna een derde van de in het verpleeghuis opgenomen patiënten een antipsychoticum voorgeschreven, vaak voor lange duur. De bijwerkingen van het gebruik van het overgrote deel van de antipsychotica bij demente ouderen zijn nog weinig onderzocht. De gunstige effecten ervan op het gedrag zijn in vergelijking met de effecten van placebobehandeling beperkt. Recente studies wijzen vrijwel alle in de richting van een verhoogde mortaliteit optredend binnen 3 maanden. De klassieke antipsychotica blijken even schadelijk dan de atypische. Het is van belang om meer terughoudend te zijn met het voorschrijven van antipsychotica bij dementie.
