Desaturatie tijdens inspanning kan bij patiënten met COPD (chronic obstructive pulmonary disease) bijdragen aan kortademigheid en verminderde inspanningscapaciteit. De longfunctie is een slechte voorspeller voor de mate van desaturatie; dit maakt een individuele assessment noodzakelijk. Zuurstofsuppletie tijdens inspanning vergroot in sterk wisselende mate de trainingstolerantie bij COPD-patiënten. Overtuigend bewijs vanuit effectonderzoek ontbreekt voor het systematisch gebruik van zuurstofsuppletie. Een besluit al dan niet te te trainen met gebruik van zuurstofsuppletie kan genomen worden op basis van de individuele respons op zuurstofsuppletie. Toepassing lijkt gerechtvaardigd indien zuurstofsuppletie bij een patiënt significant effect heeft op de trainingstolerantie. Vanuit kennis over de pathofysiologie lijkt het verstandig in ieder geval zuurstofsuppletie tijdens training toe te passen bij COPD-patiënten die bekend zijn met hartritmestoornissen of secundaire pulmonale hypertensie. Toekomstig onderzoek zal gericht moeten zijn op de selectie van patiënten die baat hebben bij training met zuurstofsuppletie.
