Behandeling van zwakbegaafde en licht verstandelijk gehandicapte plegers van seksueel misbruik

Samenvatting

The treatment of sexual abuse committed by people with a mild intellectual handicap or borderline intelligence used to be based on highly structured behaviour therapy. Recently, elements of cognitive behavioural therapy like relapse-prevention were added. This article reviews recent literature in this area. A therapy program in a secure ward is described, including its preliminary results. Cognitive behavioural therapy seems to offer a possibility for perpetrators with borderline intelligence, as part of a more comprehensive programme. Complex mental and psychosocial deficits may disturb the program to an extent that it cannot be completed. Perpetrators with a mild intellectual handicap (iq below 70) may not profit from the cognitive elements in the program.

Samenvatting

Bij de behandeling van zwakbegaafde of licht verstandelijk gehandicapte plegers van seksueel misbruik wordt veelal een directief-gedragstherapeutische ingang gekozen. Recentelijk is daar ook de cognitieve benadering aan toegevoegd, in de vorm van de delictscenario- en terugvalpreventiemethode. In dit artikel wordt een overzicht gegeven van de beschikbare literatuur op dit gebied. We beschrijven een intramuraal behandelprogramma waarvan we de eerste resultaten presenteren. Voor de groep zwakbegaafde daders die in staat is te profiteren van groepstherapie, lijkt deze methode, als deel van een meer omvattend programma, voldoende ingang te bieden voor behandeling. Ernstige psychische stoornissen en psychosociale tekorten kunnen ertoe leiden dat het programma moet worden afgebroken. De indruk bestaat dat daders met een lichte verstandelijke handicap (een iq van 50-70) onvoldoende kunnen profiteren van een cognitieve aanpak.
