Abstract
For aproaching the anterior skull base, Fisch technique (type-C) is well established (Fisch, 1984), The pterygopalatine fossa, pterygo-maxillary fissure and the nasopharynx can be approached through total and medial maxillecromies. All these especially Fisch approach, require external skin incisions and very extensive anatomical dissections.
A new surgical technique has been developed in which a lateral segmental maxillo-zygometicotomy is performed to expose the retromaxillary, peritubal and nasopharyngeat regions in which a segment of bone, consisting of maxilla and zygoma, is removed and the lesion is dealt with in the above area. Finally the segment of bone is replaced in its place and wired which maintains the contour of the face. No external skin incision is necessary. The whole procedure is performed through a modified sublabial incision (Ghosh. 1985). The anatomical dissection is much less extensive than the classical approaches. It offers a direct and excellent view to the above areas.
