Zusammenfassung
Okulomotoriusneurinome sind seltene Tumoren, die meist mit einner Augenmuskelparese symptomatisch werden. In der Literatur finden sich keine Berichte über die Exstirpation eines Neurinoms unter Erhalt der Funktion des Nervus oculomotorius. Da eine Verbesserung der inneren und äußeren Okulomotorik durch eine Operation nicht zu erwarten ist und vielmehr die Gefahr der Funktionsverschlechterung besteht, sollte eine Resektion dieses benignen, langsam wachsenden Tumors nur dann erfolgen, wenn die Raumforderungswirkung diese erfordert. Daher haben die Diagnosestellung und Befundkontrolle durch klinische und bildgebende Verfahren eine besonderen Stellenwert. Es werden im folgenden die neuroradiologischen Untersuchungen bei zwei Patientinnen dokumentiert, bei denen unter Einbezug neuroophthalmologischer Befunde und in einem Fall auch der neurochirurgischen Inspektion die Diagnose eines Okulomotoriusneurinoms gestellt wurde. Bei der neuroophthalmologischen Untersuchung spielt die Reihenfolge des Auftretens der inneren und äußeren Augenmuskelparese eine besondere Rolle. Zur Differentialdiagnose wird ein Fall eines histologisch gesicherten Okulomotoriusmeningeoms gegenübergestellt.
Summary
Oculomotor schwannomas are very rare tumors. The presenting sign is usually an ocular mobility disorder. Successful surgical removal of an oculomotor schwannoma with preserved nervous function has not been reported yet. The extirpation of the tumor cannot improve the oculomotor function but breas the risk of worsening the paresis. Therefore we believe that a surgical removal should only be considered if rendered necessary by the space-occupying effect of the lesion. In view of this concept the correct clinical and neuroradiological diagnosis and follow-up are of particular importance. We present 2 cases where the diagnosis of oculomotor schwannoma was based on neuroophthalmological and neuroradiological findings. The time course of occurrence of an internal and external ophthalmoplegia plays an important role in the neuroophthalmological examination. In one case a neurosurgical inspection was performed. A third case of an oculomotor meningioma is discussed as differential diagnosis.
