Summary

May one speak of precancerous lesion in the presence of perianal condyloma acuminata? In literature there are numerous reports of neoplastic transformation of anal condylomatosis to such an extent as these reports are comparable to extra-mammary Paget’s disease, to the giant condylomatosis of Buschke-Lowenstein and Bowen’s disease. Risk factors for malignant or premalignant change were: sexual orientation (homosexual or bisexual), HIV status and location above the dentate line. In our experience, in cases correlation between anal squamous carcinoma and condylomatosis has been established, it had occurred in 1 case, 18 months after local excision; another following radiotherapy cycle, a development at the same site of the surgical operation with metastasis in an inguinal lymph node. This latter case induces us to deem how necessary a meticulous follow-up in patients submitted to local excision of anal condylomas.

Zusammenfassung

Kann man im Zuammenhang mit perianalen Condylomata acuminata von einer präkanzerösen Läsion sprechen? In der Literatur finden sich zahlreiche Berichte über die neoplastische Transformation einer analen Condylomatosis, sofern sich diese Berichte mit dem extramammalen Morbus Paget, den riesigen Kondylomen eines Morbus Buschke-Löwenstein oder Morbus Bowen vergleichen lassen. Risikofaktoren für eine maligne oder prämaligne Veränderung waren: sexuelle Orientierung (homosexuell oder bisexuell), HIV-Status und eine Lokalisation oberhalb der Linea dentata. Wir konnten in zwei Fällen eine Korrelation zwischen einem squamösen Analkarzinom und Condylomatosis nachweisen: In einem Fall trat es 18 Monate nach Lokalexzision auf; im zweiten Fall entwickelte sich das Karzinom nach Radiotherapie an der Operationsstelle mit Metastasierung in einen inguinalen Lymphknoten. Dies hat uns bewiesen, wie notwendig eine genaue Nachuntersuchung bei Patienten mit Lokalexzision von Condylomata acuminata ist.
