Introduction
La dégénérescence maligne du parenchyme pancréatique sur pancréatite chronique calcifiante (PCC) est très rare. Elle survient chez 4% des patients au cours des 20 années qui suivent le diagnostic de la pancréatite chronique. Nous rapportons une observation d’adénocarcinome du pancréas sur PCC chez une patiente de 51 ans en soulignant les difficultés diagnostiques posées par cette complication rare, aux symptômes et à l’imagerie non spécifiques.
Observatio
Une femme de 51 ans, non alcoolo-tabagique, était hospitalisée pour douleurs épigastriques de type pancréatique associées à une diarrhée de malabsorption. L’examen clinique trouvait une sensibilité épigastrique. L’échographie et la tomodensitométrie abdominale montrait l’aspect de pancréatite chronique évoluée avec des calcifications pancréatiques intracanalaires posant ainsi le diagnostic de PCC. Le bilan étiologique était négatif. La patiente avait bénéficié d’un traitement symptomatique à base d’extraits pancréatiques et d’antalgiques, mais devant la persistance et même l’aggravation de la douleur pancréatique, elle a été confiée aux chirurgiens pour dérivation. Au cinquième jour post opératoire, la patiente avait présenté de violentes épigastralgies de type pancréatique. Les examens morphologiques avaient objectivé au niveau de la tête du pancréas, une image hétérogène. La malade a été reprise chirurgicalement et l’étude histologique de la pièce opératoire a conclu à un adénocarcinome du pancréas.
Conclusion
Le cancer du pancréas sur pancréatite chronique demeure une complication rare, le diagnostic est difficile et fréquemment tardif en raison de la non spécificité des symptômes, d’où l’intérêt de la surveillance rigoureuse et régulière de tout patient porteur de PCC probablement par une série de CA19.9 associée à des biopsies percutanées lorsque les valeurs sont augmentées ou lorsque de nouveaux symptômes apparaissent.

Introduction
The malignant degeneration of the pancreatic parenchyma on a chronic calcifying pancreatitis (CCP) is very rare. It happens in 4% of patients during the twenty years following the diagnosis of a chronic pancreatitis (CP). We report an observation of a pancreatic adenocarcinoma (PAC) with CCP in a 51-year old female patient, pointing out the diagnostic difficulties created by this rare complication with nonspecific symptoms and imagery.
Observation
A nonsmoking, nondrinking, 51-year old woman was hospitalised for epigastric pain of pancreatic type associated with malabsorption diarrhoea. The clinical diagnosis found epigastric sensitivity. The abdominal ultrasound and the abdominal tomodensitometry showed the aspect of a CP evolved with intraductal pancreatic calcifications leading hence to the diagnosis of PCC. The etiologic assessment was negative and the patient has benefited from a symptomatic treatment based on pancreatic extracts and analgesics, but with the persistence and even the worsening of the pancreatic pain, she was entrusted to a surgeon for a derivation. At the fifth postoperative day, the patient presented severe epigastric pain of pancreatic type. The morphologic examination showed at the level of the pancreas head a heterogeneous image which led to a second surgery and the histological study concluded to a PAC.
Conclusion
The pancreatic cancer on a CP is still a rare complication, the diagnosis is difficult and is frequently made late because of the unspecific symptoms; thus a rigorous follow up of patients with PCC should be instaured, probably through a series of CA19.9 associated with percutaneous biopsies when the values are increased or when new symptoms appear.
