Place de la biopsie hépatique en présence d’une maladie hépatique chronique chez l’adulte

Résumé

La biopsie hépatique demeure indispensable dans le cadre du diagnostic et du traitement des maladies hépatiques chroniques de l’adulte. Sa réalisation est néanmoins associée à des risques de morbidité et de mortalité qui obligent le clinicien à définir cette place avec intelligence et en collaboration étroite avec le pathologiste. Dans cet article, la valeur ajoutée de l’analyse histologique du foie sera discutée dans le cadre des différentes pathologies hépatiques chroniques. L’apport essentiel de cette analyse histologique se situe dans sa contribution à la stadification de la maladie et à son pronostic sans oublier la possibilité de la mise en évidence d’une deuxième pathologie potentiellement ignorée par le clinicien. Le présent article est dévolu à la place de la biopsie hépatique en présence d’une maladie hépatique chronique chez le patient adulte non transplanté. La place de la biopsie hépatique dans le contexte d’une tumeur hépatique ne sera pas envisagée.

Summary

Hepatic biopsy remains indispensable in the framework of diagnosis and treatment of chronic hepatic disease in adults. It is, nevertheless, associated with a risk of morbidity and mortality that obliges the clinician to define this role with intelligence and in direct collaboration with the pathologist. In this article, the added value of the histological analysis of the liver will be discussed in the framework of different chronic hepatic pathologies. The main use of the histological analysis is its contribution to elucidating the stage of the disease and its prognostic without forgetting the possibility of revealing a second pathology potentially unknown to the clinician. The present article focuses on the role of hepatic biopsy in the presence of chronic hepatic disease in the adult, nontransplanted patient. The role of hepatic biopsy in the context of a hepatic tumour will not be discussed.
