Valeur comparée de la wirsungographie endoscopique de l’artériographie abdominale et de l’échotomographie en pathologie pancréatique

Comparative estimation of endoscopic wirsungography, abdominal arteriography and ultrasonography in the diagnosis of pancreatic diseases

Résumé

Afin d’apprécier la valeur diagnostic de la wirsungographie endoscopique, de l’échotomographie et de l’artériographie hypersélective, 104 wirsungographies, 64 artériographies et 103 échotomographies réalisées à la recherche d’affections pancréatiques ont été comparées. Les pancréatites chroniques ont été correctement diagnostiquées dans 86,36% des cas par wirsungographie, dans 66,66% par artériographie et dans 90,90% par échotomographie. Cette dernière méthode est également très sûre pour la recherche de kystes (93,33% des cas). 92,85% des cancers du pancréas ont été diagnostiqués par wirsungographie, 86,66% par artériographie, 84,41% par échotomographie. Cette étude permet une approche précise des affections pancréatiques: dans un but diagnostique l’échotomographie et la wirsungographie endoscopique sont complémentaires alors que l’artériographie est surtout utile avant un acte chirurgical.

Summary

In order to evaluate the diagnostic accuracy of endoscopic wirsungography, hyperselective angiography and ultrasonography, 104 wirsungograms, 64 arteriograms and 103 echotomograms performed for pancreatic investigation were studied. Chronic pancreatitis were correctly diagnosed by wirsungography in 86,36% of cases, by arteriography in 66,66% of cases, by ultrasonography in 90,90%. Pancreatic cysts are easily detectable by ultrasonography (93,33% of cases). Pancreatic cancers were found by wirsungography in 92,85% of cases, by arteriography in 86,66% of cases, by ultrasonography in 84,41% of cases. This comparative study allows a precise approach of pancreatic diseases: for a diagnostic purpose ultrasonography and endoscopic wirsungography are two complementary methods whereas arteriography is especially useful before surgery.
