Résumé
Les auteurs rapportent deux cas d'adénocarcinome œsophagien développé sur un épithélium de Barrett avec hernie hiatale. Ils discutent le résultat de la confrontation entre les données de l'endoscopie, des biopsies étagées multiples, et de l'examen de la pièce opératoire. Cette analyse conduit à adopter une attitude prospective en présence d'un endobrachyœsophage ou de lésions de métaplasie gastrique au niveau de l'œsophage.
Summary
Esophageal adenocarcinoma arising in a Barrett's epithelium were found in two cases of hiatal hernia complicated with peptic esophagitis. Data of endoscopy, multiple biopsies and surgical specimens dissection are discussed. The authors propose a prospective periodic screening of patients with columnar lined epithelium for early diagnosis of dysplastic changes and adenocarcinoma.
