Accuracy and limits of the diagnosis of primary lymphoma on gastric endoscopic bipsies

Résumé

Chez les patients porteurs de lymphome gastrique primitif, l'efficacité diagnostique de la biopsie endoscopique préopératoire dépend du nombre et de la profondeur des prélèvements. En combinant une technique de prélèvements nombreux en sites multiples, le rendement diagnostique de notre série a été de 19 patients positifs sur 24 tandis que le diagnostic de lymphome était suspecté chez deux autres patients et que trois cas seulement sont restés négatifs.

Si la répartition des types histologiques de lymphome entre faible et haut degré de malignité apparaît réalisable sur notre matériel endoscopique, ce dernier est trop réduit et trop superficiel pour permettre l'utilisation dans de bonnes conditions d'une classification moderne des lymphomes (classification de Kiel).

Summary

The diagnosis of primary gastric lymphoma on endoscopic preoperatory biopsies was made in 19 patients out of 24 while the diagnosis was suspected in 2 patients; 3 cases only remained negative. This high diagnostic yield was obtained by the practice of numerous biopsies in multiple lesional sites.

It was rather easy to discriminate, on our endoscopic material, between high grade and low grade malignancy lymphomas. However the use of a modern classification of lymphomas (Kiel classification) could not be applied in every case owing to the small size of the biopsies.
