Conclusions
L’entéroscopie poussée par simple et double voies est entrée dans la pratique quotidienne pour explorer l’intestin grêle. Elle supplante l’utilisation de la sonde entéroscopie dans la mesure où, même si elle est incomplète, elle permet la résolution de la majorité des situations cliniques pour lesquelles l’examen a été demandé. D’autre part il faut garder à l’esprit que 30 à 40 % des lésions retrouvées au décours de ces entéroscopies sont localisées dans les segments du tube digestif qui auraient pu être examinés à l’aide d’endoscopes classiques. Les efforts doivent donc porter dans deux directions: l’un, immédiat, qui dépend de l’opérateur est d’examiner dans de bonnes conditions techniques et avec attention les patients soumis à l’exploration endoscopique initiale, l’autre qui est en cours, est d’améliorer le matériel endoscopique mis à notre disposition dans le sens d’un compromis rigidité/flexibilité différenciée adaptée à la progression dans l’intestin grêle.
Conclusion
One and two-way push enteroscopy is now a common technique to explore the small bowel. It replaces the use of the sonde enteroscope since, even when it is incomplete, it allows the resolution of the majority of clinical cases for which this examination has been prescribed. In addition, one has to bear in mind that 30 to 40 % of the lesions found during this type of enteroscopy are localized in parts of the digestive tract that could have been explored with classical endoscopes. Current efforts should then be aimed in two directions: the first with immediate effect, depends on the operator and consists of a careful examination in good technical conditions of the patients who have undergone initial endoscopic exploration; the other which is in progress, is to improve the endoscopic material available by finding the right compromise of rigidity/differential flexibility suitable to progression inside the small bowel.
