Traitement non chirurgical des complications des pancréatites chroniques

Conclusion

Au cours des dernières années, des traitements endoscopiques de la pancréatite chronique, ayant pour but de rétablir l’écoulement du suc pancréatique dans le duodénum, ou pour traiter certaines complications, ont été proposés. Certaines techniques, comme l’intubation pancréatique, sont toujours en cours d’évaluation. Le drainage endoscopique des kystes pancréatiques donne des résultats intéressants, faisant de cette méthode une bonne alternative au traitement chirurgical. Le traitement endoscopique des sténoses de la voie biliaire principale peut être proposé chez des malades sélectionnés.

Tous ces traitements doivent être réalisés par des endoscopistes expérimentés, dans des centres de référence, et toujours après discussion médico-chirurgicale.

Conclusion

During recent years, endoscopic treatments of chronic pancreatitis, aiming to restore the flow of pancreatic juice into the duodenum or to treat some complications, have been proposed. Some techniques, such as pancreatic stenting, are still under evaluation. Endoscopic drainage of pancreatic cysts gives interesting results, making this method a good alternative to surgery. Endoscopic treatment of stenosis of the main bile duct can be proposed in selected patients.

All these treatments should be performed by experienced endoscopists, in reference centers and always after discussion with the surgical team.
