RÉsumÉ
L’introduction de fibroscope souple dans l’exploration digestive a permis ces dernières années de préciser l’indication de l’endoscopie œso-gastro-duodénale chez les enfants porteurs d’une hypertension portale. L’endoscopie œso-gastro-duodénale s’est avérée l’examen le plus performant pour la mise en évidence des varices œsophagiennes; elle permet de préciser le risque hémorragique de l’hypertension portale; en phase hémorragique, elle précise l’origine du saignement; enfin, après shunt porto-systémique, elle permet de contrôler la régression des signes d’hypertension portale.
Summary
The use of soft fibroscopes in exploration of the digestive tract over the past few years, has clarified the indication of eso-gastro-duodenal fibroscopy in children suffering from portal hypertension. Eso-gastro-duodenal endoscopy has proven to be the best examination if one wants to show the existence of esophageal varicosities; it evaluates the risk of hemorrhage in cases of portal hypertension; if there is hemorrhage it enables us to pinpoint its origin; finally once the porto-systemic shunt is in position, it enables us to check up on the regression of portal hypertension signs.
