Place de l’échoendoscopie dans le bilan d’extension du cancer de l’œsophage

Résumé

Le bilan préthérapeutique du cancer de l’œsophage a été l’une des premières indications de l’échoendoscopie haute (EE). Dès le début des années 1980 en effet, des résultats supérieurs à ceux des autres techniques d’imagerie disponibles ont été démontrés. Ils restent encore aujourd’hui inégalés tandis que divers progrès techniques (minisondes, hautes fréquences, cytoponctions échoguidées) ont accru la précision de l’EE et étendu son usage [1, 2]. A l’heure des traitements combinés et des évaluations pronostiques, l’EE voit donc sa place confirmée dans les principaux arbres de décisions et protocoles de traitement du cancer de l’œsophage.

Summary

The pretherapeutic assessment of oesophageal cancer was one of the first indications of upper endoscopic ultrasonography (EUS). As a matter of fact, from the early 80’s better results than those obtained with other imaging techniques have been demonstrated. They are still unequalled whereas various technical advances (miniprobes, high frequencies, EUS FNA) have increased the accuracy of EUS and widespread its use[1, 2]. At the time of combined treatments and prognostic assessments, the EUS keeps its confirmed place in the main decision trees and protocols for the treatment of oesophageal cancer.
