Abstract
Die Erfassung des Konzeptes der Lebensqualität gewinnt in medizinischen Interventionsstudien immermehr an Bedeutung. Dargestellt werden psychometrische Analysen der deutschen Version des Medical Outcome Study Health Surveys in der Short Form mit 36 Items (MOS-SF-36), der ein generisches Instrument zur Erfassung krankheitsübergreifender Lebensqualität ist. Die Analysen stützen sich auf 1378 Patientendaten zum Zeitpunkt vor der Intervention Akupunktur bei verschiedenen Diagnosen sowie auf 978 Patientendaten zum Zeitpunkt nach der Behandlung. Es zeigte sich, dass der Fragebogen als reliabel zu bezeichnen ist, wobei die Skala Schmerz aus dem Rahmen fällt. Es fanden sich relativ starke Boden- und Deckeneffekte, die vermutlich aufgrund mangelnder Antwortabstufungen zustande kommen, so dass hier Verbesserungsbedarf besteht, der sich auch auf die Anzahl fehlender Werte sowie auf die ungenügende Normalverteilung der Skalen auswirken könnte. Die angenommene Faktorenstruktur findet sich auf Itemebene wieder, wobei eine konfirmatorische Faktorenanalyse die Annahme zweier interkorrelierter Gesundheitsdimensionen sowie die Bildung eines Summenfaktors nahelegt. Ein Vergleich mit anderen psychometrischen Analysen der deutschen Version von Zwingmann et al (1998) zeigt, dass in beiden Analysen vergleichbare Ergebnisse gefunden werden konnten.
Abstract
We report psychometric analyses of the German version of the SF 36 health survey, a widely used instrument to assess generic health status or illness related quality of life. Our analysis is based on questionnaires of patients suffering from a variety of diseases, mainly pain syndromes, who sought acupuncture treatment, before commencement of (n=1378) and after (n = 978) treatment. We found good reliability of the scales except for the scale “bodily pain”. There are considerable floor and ceiling effects, as well as missing data. This is presumably due to the fact that many items are only dichotomous. The purported factorial structure of the questionnaire can be approximately reproduced across items and scales. A confirmatory factor analysis confirms either two intercorrelated factors “mental health” and “physical health ” or one general factor. Our results closely resemble a recently published analysis of Zwingmann et al.(1998) in back-pain patients. We conclude that the scale is reliable and psychometrically sound, with room for improvement. Improvement could be made in adjusting the scaling of items to a common 5 point Likert system. The factorial structure implies either two interrelated factors or, as a viable alternative, one single factor.
