Zusammenfassung

Grundlagen:Longo’s Technik der Stapler-Mukosektomie in der Behandlung 3- bis 4-gradiger prolabierender Hämorrhoidalknoten findet auch in Österreich breite Anwendung. Nach anfänglicher Euphorie bezüglich des neuen nahezu schmerzlosen Verfahrens werden nach und nach Berichte über schwerwiegende Komplikationen bekannt. Die Ursache für postoperativ über Wochen bis Monate bestehende funktionelle Probleme wie Dranginkontinenz, verbunden mit brennenden Schmerzen im Bereich des Rektums trotz korrekt ausgeführter Longo-Technik, ist bis heute noch unbekannt.

Methodik: Prospektiv randomisierte Multicenter-Studie an 100 Patienten mit prolabierenden Hämorrhoidalknoten Grad III und IV.

Ergebnisse: Vergleichende Studien mit der konventionellen Hämorrhoidektomie nachMilligan-Morgan sind bisher in Frankreich (3) und Schweden (10) durchgeführt worden. Erste Ergebnisse wurden vonM. Rowsell und B. J. Mehigan (7, 13) publiziert.

Schlußfolgerungen: Untersuchungen über funktionelle Langzeitergebnisse stehen noch aus. Wir glauben, daß es auch in Österreich wichtig und sinnvoll ist, Vorteile und Risiken der neuen Technik in randomisierten Studien zu untersuchen. Wir möchten alle interessierten Koloroktologen zur Teilnahme an der Studie einladen.

Summary

Background:Longo’s stapling technique has become very popular in the therapy of 3th and 4th stage hemorrhoids in Austria. The first euphory because of the nearly painless procedure was followed by critical reports about serious complications. Functional problems with incontinence and burning pains in the rectum may persist for several months. In all casesLongo’s procedure was performed correctly. The causality for these functional problems is unknown.

Methods: Prospective randomised multicenter study in 100 patients with haemorrhoidal prolaps stage III and IV.

Results: Randomised studiesLongo vs.Milligan-Morgan are reported from France (3) and Sweden (10).M. Rowsell undB. J. Mehigan (7, 13) published first clinical results.

Conclusions: There are no reports on functional long-term results. In our opinion randomised studies to evaluate the advantages and risks of this new technique are also important in Austria. We would like to invite all coloproctologists to participate in our study.
