Samenvatting
Sinds 2011 bestaat er een verplichting tot deelname aan de landelijke onlineregistratie voor radicale cystectomieën in het kader van spierinvasief blaascarcinoom. Vanaf 1 januari 2012 dienen alle patiënten die een radicale cystectomie ondergaan, ingevoerd te worden in deze database. In totaal zijn in het jaar 2010 de data van 535 patiënten ingevoerd door 55 ziekenhuizen. In 2011 betrof het 749 patiënten door 54 ziekenhuizen.
Voor dit artikel zijn de gegevens uit 2011 gebruikt van 719 van de 749 patiënten (96%), van wie de patiëntgegevens volledig ingevoerd waren. Het betreft 210 vrouwen (29%) en 509 mannen (71%). In 22 ziekenhuizen worden jaarlijks 10 of minder cystectomieën uitgevoerd en in 9 centra meer dan 20. De 30-dagenmortaliteit varieert tussen de 2,0% en 2,6%. In academische ziekenhuizen wordt vaker een orthotope neoblaas aangelegd.
Met de resultaten uit de database is kwaliteitscontrole mogelijk op grond van de criteria van de Werkgroep Oncologische Urologie (WOU). Audits kunnen de kwaliteit van de dataverzameling verder verbeteren.
Summary
Radical cystectomy, in the Netherlands, in 2011. Evaluation of the national database
Patients undergoing radical cystectomy in the Netherlands, with the intent to cure, for muscle-invasive transitional-cell carcinoma of the bladder are registered in an online database. From January 1st, 2012, registration in the database is obligatory. In 2010 55 hospitals entered 535 patients, for the year 2011 this was 749 patients by 54 hospitals. Data from 719 (210 women and 509 men) out of 749 patients for the year 2011 were complete and were evaluated. Twenty-two hospitals performed less than 10 cystectomies per year and 9 centers more than 20 per year. The overall 30-days post-operative mortality rate varied between 2.0 and 2.6%. In academic hospitals the proportion of orthotopic neobladders was higher. Evaluation of the database produces valuable data which can be used for quality control. Audits will further increase the quality of the data.
