Résumé
Introduction
Les unités cognitivocomportementales (UCC) sont dédiées à la prise en charge des patients atteints de maladie d’Alzheimer et maladies apparentées présentant une aggravation des troubles du comportement. La participation du pharmacien-clinicien à l’optimisation de la thérapeutique, en collaboration avec les équipes médicale et paramédicale, est essentielle dans la prévention du risque iatrogène dans cette population. Notre objectif est de décrire les interventions pharmaceutiques (IP) transmises par le pharmacien-clinicien au sein de l’UCC de notre établissement.
Méthodologie
Tous les patients hospitalisés au cours de l’année 2011 ont été inclus. Le pharmacien-clinicien effectuait l’analyse pharmaceutique à l’admission, de façon hebdomadaire et participait à la réunion pluridisciplinaire hebdomadaire.
Résultats
Au total, 97 patients ont été inclus dans l’étude. Sur les 560 ordonnances analysées, 190 IP et trois informations ont été formulées. Le taux d’acceptation est de 77,9 %. Parmi les principaux problèmes identifiés, 39,9 % concernaient une non-conformité du choix du médicament aux référentiels. Une substitution était proposée dans 32 % des cas.
Conclusion
Cette étude montre la place essentielle du pharmacien-clinicien au sein de l’équipe pluridisciplinaire afin d’optimiser la prise en charge médicamenteuse et de limiter la iatrogénie chez les patients atteints de troubles neurocognitifs.
Abstract
Purpose
The cognitive-behavioral units (UCC) are dedicated to Alzheimer’s disease patients with a worsening of behavior disorders. The aim is to describe pharmaceutical interventions performed by the clinical pharmacist during prescriptions analysis.
Methodology
All patients hospitalized in the UCC (Hospices de Lyon, France) during 2011 were included. The clinical pharmacist performed prescription analysis and participated in multidisciplinary review.
Results
Ninety-seven patients were included in the study. Among the 560 analyzed prescriptions, 190 pharmaceutical interventions were performed by clinical pharmacist and 148 were accepted (77.9%). Most frequently detected drug-related problems were nonconformity to guidelines or contraindication (39.9 %), dose adjustment (19.5%), and inappropriate administration route (17.6%). A substitution was proposed for 32% of pharmaceutical interventions.
Conclusion
This study shows the impact of clinical pharmacist integrated to the multidisciplinary staff in order to improve drug prescribing and decrease iatrogenic risk in elderly.
