Résumé
Le diagnostic de la leucémie aiguë (LA) est fondé sur des arguments cliniques et biologiques. L’objectif de l’étude est de décrire les caractéristiques cytologiques de 814 cas de LA colligés au laboratoire d’hématologie du centre hospitalier universitaire Ibn-Rochd de Casablanca entre le 1er janvier 2004 et le 31 juillet 2007. L’examen morphologique de la moelle osseuse et la réaction à la myéloperoxydase ont permis de classer les LA: 1) 64 % des LA sont de type myéloblastique (LAM); 2) 30 % des LA sont de type lymphoblastique (LAL); 3) 6 % des LA sont difficiles à classer selon les critères du groupe franco-américanobritannique (FAB). Dans cette série, 81,6 % des cas de LAM sont observés chez les adultes avec un sex-ratio M/F de 1,05; pour les LAL, 67,3 % des cas sont diagnostiqués chez les enfants avec un sex-ratio M/F de 1,2. Selon la classification FAB, les LAM1 et LAM2 sont les plus fréquentes, respectivement 31 et 28 %, pour les formes lymphoblastiques, le type 2 est prédominant avec 75 %.
Abstract
The diagnosis of AML is based on the clinical and biological arguments. The purpose of this study is to describe cytological characteristics of 814 cases of acute leukemia collected at the Laboratory of Haematology, University Hospital Ibn-Rochd of Casablanca between 1st January 2004 and 31 July 2007. Morphological examination of bone marrow and the reaction to myeloperoxidase were used to classify acute leukemias: 1) 64% of acute leukemias are the myeloid type (AML); 2) 30% of acute leukemias are the lymphoblastic type (ALL); 3) 6% of acute leukemias are difficult to classify according to criteria of the FAB group. In this series, 81.6% of AML cases are seen in adults with a sex ratio M/F of 1.05, for ALL, 67.3% of cases are diagnosed in children with a sex ratio M/F of 1.2. According to FAB classification, and the LAM1 and LAM2 are most frequent, respectively 31% and 28 %, for lymphoblastic form, type 2 is predominant with 75%.
