Comparison of the Efficacy and Safety of Early Rule-Out Pathways for Acute Myocardial Infarction. Circulation. 2017;135:1586-1596.

Background:Guidelines from the European Society of Cardiology (ESC) support early testing with high-sensitivity troponin in patients presenting with chest pain to the emergency department. Chapman et al. from the Royal Infirmary of Edinburgh, United Kingdom, compared two pathways that utilize high-sensitivity troponin: ESC pathway and the previously validated High-STEACS pathway. The ESC pathway rules out myocardial infarction (MI) when troponin is less than 95th percentile at presentation in those with symptoms >6 hours and at 3 hours in those with symptoms less than 6 hours or if it did not change from baseline. The High-STEACS pathway rules out MI when trop <5 ng/L in those with symptoms ≥2 hours. In patients with symptoms <2 hours or troponin ≥5 ng/L but less than 99th percentile, the test is repeated at 3 hours and MI is ruled out if trop is...
