Zusammenfassung
Die ARVC ist eine wichtige Differenzialdiagnose bei Patienten mit ventrikulären Tachykardien ohne evidente strukturelle Herzerkrankung. Die Diagnosestellung orientiert sich weiterhin an den seit 1994 gültigen Task-Force-Kriterien. Eine allgemein akzeptierte Modifikation dieser Kriterien unter Berücksichtigung moderner bildgebender Verfahren existiert bisher nicht. In der vorliegenden Kasuistik wird über einen 60-jährigen Patienten mit anhaltender ventrikulärer Tachykardie berichtet. Die MRT lieferte bei diesem Patienten entscheidende Hinweise für das Vorliegen einer ARVC trotz des untypisch hohen Alters bei Erstmanifestation. Unter Einsatz eines 3-dimensionalen Mappingsystems konnte in einer elektrophysiologischen Untersuchung die Lokalisation des pathologischen Befundes im inferioren RV bestätigt, dessen Eigenschaft als morphologisches Substrat der ventrikulären Tachykardie nachgewiesen und eine Katheterablation erfolgreich durchgeführt werden. Die Kasuistik illustriert die zunehmende Bedeutung bildgebender Verfahren für Diagnostik und Therapieplanung bei Patienten mit ventrikulären Tachykardien.
Abstract
Arrhythmogenic right ventricular cardiomyopathy (ARVC) represents an important differential diagnosis in patients with ventricular tachycardia in the absence of evident structural heart disease. Diagnosis of ARVC is currently based on task force criteria established in 1994. However, a commonly accepted modification of these criteria including recent imaging techniques is still missing. In this case report, a 60-year-old patient with sustained ventricular tachycardia is presented. MRI provided crucial findings for the diagnosis of ARVC despite the atypically old age. An electrophysiological study using three-dimensional mapping confirmed localization of pathologic MRI findings in the inferior RV. This region was identified as the morphological substrate for clinical ventricular tachycardia and catheter ablation was successfully performed. This case report illustrates the increasing importance of imaging techniques for diagnosis and guidance of therapy in patients with ventricular tachycardia.
