Pancreatic cancer in Burundi, experience in Kamenge University Hospital

Résumé

Aucune étude n’avait été faite sur le cancer du pancréas dans notre pays. Le but était de déterminer les aspects épidémiologiques, cliniques et thérapeutiques de ce cancer. C’est une étude descriptive, rétrospective sur 15 ans (1994 à 2009), menée dans le service de gastro-entérologie et de chirurgie du CHU de Kamenge. Durant cette période, nous avons colligé 32 cas d’adénocarcinome canalaire du pancréas: 23 hommes (71,9%) et 9 femmes (28,1%). 4 cas de cystadénomes ont été exclus de l’étude. L’âge moyen était de 50,3 ans avec des extrêmes de 38 et 66 ans. Les principaux facteurs de risque retrouvés étaient le tabagisme chez 19 patients (59,4%) et l’alcoolisme chronique chez 15 malades (46,8%). L’ictère, l’amaigrissement et les douleurs abdominales ont été les symptômes cliniques dominants qui ont conduit à la réalisation d’une échographie chez les 32 patients (100%). Les tumeurs étaient principalement situées dans la tête du pancréas chez 25 malades (78,1%). Lors de la laparotomie, 29 malades (90,6%) avaient des métastases. 3 patients (9,4%) ont eu une duodénopacréatectomie céphalique, 16 malades (50%) ont eu une double dérivation bilio-digestive et gastro-jéjunale et 2 patients (6,2%) ont eu une dérivation bilio-digestive seule. Les 11 patients (34,4%) restants ont eu une laparotomie sans geste thérapeutique. La chimiothérapie et le drainage endoscopique n’ont pas été utilisés.

Abstract

The aim of the study was to determine the epidemiological, clinical and therapeutic aspects of pancreatic cancer. It is a descriptive and retrospective study over 15 years (1994 to 2009) carried out in the gastroenterology and Surgery departments of Kamenge University Hospital. During that period, a total of 32 cases of adenocarcinoma have been collected. 23 males (71,9%) and 9 females (28,9%). The mean age was 50,3 years (range: 38–66). As main risk factors, the tobacco addiction was found in 19 patients (59,4%) and chronic alcohol in 15 patients (46,8%). Jaundice, weight-loss and abdominal pain were the main clinical symptoms, which led to the tomography exam for the 32 patients (100%). The tumours were located in the head of the pancreas in 25 patients (78,1%). During laparotomy, 29 patients (90,6%) had metastasis. As treatment, 3 patients (9,4%) had a cephalic duodenopancreatectomy, 16 patients (50%) had a double biliodigestive and gastro-jejunal bypass and 2 patients (6,3%) had bilio-digestive bypass. The other 11 patients (34,4%) had only diagnostic laparotomy. No patient had chemotherapy or endoscopic biliary drainage.
