Zusammenfassung
Über die Stabilität von Antiepileptika im menschlichen Blut liegen bislang keine systematischen Untersuchungen vor. Um mögliche Einflüsse äußerer Faktoren (Transport, Temperatur und -schwankungen) nachzuweisen, wurden Konzentrationen der Antiepileptika Lamotrigin, Valproat, Primidon, Phenobarbital, Phenytoin und Carbamazepin in gespikten Serum-, Plasma- und Vollblutproben nach 24-, 48-stündiger, 7- und 30-tägiger Lagerung bei Raumtemperatur, +4°C und −20°C bestimmt. Hierzu wurden jeweils 3 Wirkspiegel im unteren und oberen therapeutischen sowie im toxischen Bereich (2,5-Fache des oberen therapeutischen Bereichs) eingesetzt. Um den Effekt von Temperaturschwankungen zu erfassen, wurden Proben 5-mal eingefroren und wieder aufgetaut. Die Messungen erfolgten mithilfe der Hochleistungsflüssigkeitschromatographie; lediglich Valproinsäure wurde im Immunoassay bestimmt. Während die Konzentrationen von Lamotrigin, Valproinsäure, Phenytoin und Carbamazepin in allen Matrizes stabil waren, zeigten Primidon und Phenobarbital nach 30 Tagen teilweise starke Abnahmen. Von den bei Raumtemperatur gelagerten Proben waren einige verdorben. Die Notwendigkeit der Einhaltung der Empfehlungen der Arbeitsgemeinschaft für Neuropsychopharmakologie und Pharmakopsychiatrie (AGNP) zu Probenlagerung und -versand (Serumproben +4°C bis 24 h, sonst Aufbewahrung bei −20◦C) wird durch diese Arbeit bestätigt.
Abstract
The stability of antiepileptic drugs in human blood has not been evaluated systematically. In order to examine possible influences of external factors (transportation, temperature, and temperature changes), the concentrations of the antiepileptic drugs lamotrigine, valproate, primidon, phenobarbital, phenytoin, and carbamazepine were determined in spiked serum, plasma, and whole blood samples after 24 hours, 48 hours, 7 days, and 30 days with the samples having been stored at room temperature (20°C), 4°C, and −20°C. Three active dosage levels were used in each case: lower and upper therapeutic levels as well as a toxic level (2.5-fold of the upper therapeutic level). In addition, the samples were frozen and thawed five times in order to determine the effects of temperature changes. All drugs except valproate were evaluated using HPLC; valproic acid was measured using an immunoassay technique. While the concentrations of lamotrigine, valproate, phenytoin, and carbamazepine were stable in all matrices, the concentrations of primidon and phenobarbital were markedly decreased (>15–24% after 7 days or in some cases even after 24 hours). Due to decomposition, some of the samples stored at room temperature were not usable after 30 days. This work confirms the current recommendations from the working group on neuropsychopharmacology (Arbeitsgemeinschaft für Neuropsychopharmakologie und Pharmakopsychiatrie, AGNP) that serum samples may be stored and shipped at 4°C for up to 24 hours, otherwise samples should be stored at −20°C.
