Zusammenfassung
Es wird über den Fall eines 76-jährigen Patienten berichtet, der den ersten Schock seiner tragbaren Defibrillatorweste erhalten hat. Aufgrund einer Endokarditis war 4 Wochen zuvor die vor Jahren bei fortgeschrittener systolischer Herzinsuffizienz implantierte kardiale Resynchronisationstherapie mit integriertem Defibrillator (CRT-D) explantiert worden. Zum Aufnahmezeitpunkt wartete der Patient auf die Reimplantation eines neuen Geräts. Das erhöhte Risiko für das Auftreten eines plötzlichen Herztods in Folge von Kammerflimmern oder von Kammertachykardien wurde zwischenzeitlich mit der tragbaren Defibrillatorweste überbrückt.
Die elektrokardiographische (EKG-)Dokumentation des Ereignisses zeigte einen inadäquaten Schock bei Vorhofflimmern. Inadäquate Schocks einer tragbaren Defibrillatorweste sind selten, außerklinisch ist es jedoch praktisch unmöglich zwischen einem adäquaten oder inadäquaten Schock zu unterscheiden. Die Autoren empfehlen daher bei allen Patienten, die einen ersten Schock durch die tragbare Defibrillatorweste erhalten haben, eine kardiologische Vorstellung unter bis dahin fortgesetztem EKG-Monitoring. Die weitere Therapie richtet sich nach der dem Schock zugrundeliegenden Rhythmusstörung, wobei im vorliegenden Fall bei tachykardem Vorhofflimmern eine Behandlung mit Amiodaron erfolgte.
Abstract
The case of a 76-year-old man with known systolic heart failure who received the first shock by his wearable cardioverter defibrillator (WCD) is reported. Due to endocarditis 4 weeks previously, the integrated defibrillator (CRT-D) that had been implanted several years ago for cardiac resynchronization therapy in advanced systolic heart failure had to be removed. At the date of admission, the patient was still waiting for reimplantation. The elevated risk for sudden cardiac death until reimplantation was bridged by a WCD.
Electrocardiographic (ECG) documentation of the event showed an inappropriate shock following atrial fibrillation. Inappropriate WCD shocks are rare; nevertheless, out-of-hospital there are hardly any possibilities to discriminate between appropriate or inappropriate shocks. We therefore suggest that all patients who received a first shock by a WCD should contact a cardiologist for continued ECG monitoring. Further therapy depends on the underlying tachycardia; in this case atrial fibrillation was treated with amiodarone.
