Diagnostic and interventional endoscopic therapy of cholangiocarcinoma

Zusammenfassung

Aufgrund der speziellen Tumoreigenschaften und der steigenden Inzidenz in den westlichen Ländern gibt es zukünftig in der Diagnostik und Therapie von Cholangiokarzinomen noch viele Herausforderungen. Jüngste Fortschritte in der Bildgebung und den endoskopischen Techniken zeigen vielversprechende Erfolge. Hauptansatz der aktuellen Forschung und der laufenden Studien ist die Verbesserung auf folgenden Feldern: Tumorfrühdiagnostik, histologische Sicherung, Resektabilitätsprüfung und adäquate Angebote zur Lebensverlängerung in der Palliativsituation. Dieser Artikel gibt einen kritischen Überblick der für die Cholangiokarzinome aktuell zur Verfügung stehenden diagnostischen und therapeutischen Mittel.

Abstract

Due to special tumor characteristics and an increasing incidence in western countries there will be many challenges in the diagnosis and treatment of cholangiocarcinoma in the future. Recent progress in imaging and endoscopic technologies has shown promising success. The main approach of current research and ongoing clinical studies is improvement in the fields of early tumor detection, histological confirmation, assessment of resectability and adequate options in offering palliative treatment concepts to increase life expectancy. This article highlights and critically discusses the currently available means for the diagnostic evaluation und therapeutic management of cholangiocarcinoma.
