Zusammenfassung
Neuroendokrine Tumoren (NET, vormals „Karzinoide“) des Ileums und Jejunums sind benigne oder unterschiedlich agressiv wachsende maligne Tumoren. Sie treten oft multipel auf. Koinizidierende nichtneuroendokrine Karzinome des GI-Trakts sind häufig. Symptome entstehen durch regionale Lymphknotenmetastasierung, Durchblutungsstörungen des Darms, Raffung des Mesenteriums und ggf. Hormonbildung. NET der Appendix sind meistens klein, liegen an der Appendixspitze und manifestieren sich häufig als „Appendizitis“. Die Therapie ist grundsätzlich chirurgisch, da dieses Vorgehen die einzige Möglichkeit zur Heilung darstellt und bei symptomatischem und nicht in sano resezierbarem Leiden eine sinnvolle Palliation ermöglicht. Durch das langsame Wachstum und die guten Überlebenszeiten sind auch wiederholte palliative Eingriffe gerechtfertigt. Appendix-NET <2 cm sind in der Regel durch Appendektomie saniert. Die operative Manipulation von NET kann eine „Karzinoidkrise“ mit lebensbedrohlicher Hypo-/Hypertension und Bronchospasmus hervorrufen. Narkose und Operation müssen daher unter dem Schutz von Somatostatin oder seinen Analoga durchgeführt werden.
Abstract
Neuroendocrine tumours (NET) are slowly developing benign or malignant lesions. Multiple manifestations and coincident non-neuroendocrine carcinomas of the GI tract are common. Symptoms arise from lymph node metastases, compromised vascularisation, desmoplastic reaction and hormonal activity. A surgical approach and repetitive procedures are justified due to the specific biological features of this tumour, either for cure or for reasonable palliation. NET of the appendix smaller 2 cm are usually cured by simple appendectomy. Life-threatening effects due to “carcinoid crisis” during surgery should be prevented by prophylaxis using somatostatin or one of its derivatives.
