ESM 2. Altered brain anatomy with hydrocephalus (dilated lateral and third ventricles, with EVD tip positioned in the anterior horn of the left lateral ventricle). In the posterior horns, the two hyperechogenic endovascular vegetations can be easily appreciated

ESM 1. Daily TCCDs performed on the right temporal window showed persistent enlargement of both lateral ventricles and third ventricle. The ventricles were dilated in both the horizontal and vertical plane. TCCD enabled measuring the size of the ventricles: lateral ventricles [vertical diameter > 4 cm (Panel A) and horizontal diameter > 2.5 (panel B) bilaterally], and third ventricle (vertical diameter > 1 cm (panel 1; horizontal diameter > 2 cm (panel B). Panel C shows in details the dilated posterior horns of the lateral ventricles (horizontal diameters > 2.5 cm bilaterally)
