Kutane Nebenwirkungen einer Hydroxyurea-Therapie bei Polycythaemia vera

Zusammenfassung

Wir berichten über eine 68-jährige Patientin mit Polycythaemia vera, die seit 8 Jahren mit Hydroxyurea therapiert wird. Sie entwickelte schmerzhafte Ulcera crurum sowie multiple hyperkeratotische aktinische Keratosen und ein Plattenepithelkarzinom. Nach Absetzen der Therapie mit Hydroxyurea heilten die Ulzerationen innerhalb von 2 Monaten ab. Die aktinischen Keratosen und das Plattenepithelkarzinom wurden mittels Kryotherapie bzw. Exzision behandelt. Eine Vielzahl an kutanen Nebenwirkungen inklusive schmerzhaften Unterschenkelulzera und Plattenepithelkarzinomen kann mit einer Hydroxyurea-Therapie assoziiert sein. Angesichts dieser Vielzahl von unerwünschten Nebenwirkungen im Zusammenhang mit einer langfristigen Hydroxyurea-Therapie sollte sichergestellt werden, dass Ärzte wie Patienten sich der spezifischen Risiken der Behandlung bewusst sind. Patienten unter einer Hydroxyurea-Therapie sollten engmaschig von einem Dermatologen untersucht werden, um rechtzeitig kutane Nebenwirkungen zu erkennen und zu behandeln.

Abstract

A 68-year-old women with polycythemia vera was treated with hydroxyurea for 8 years and developed painful ulcers on her lower legs, multiple hypertrophic actinic keratoses and a squamous cell carcinoma. After discontinuing hydroxyurea therapy the leg ulcers resolved within 8 weeks. The hypertrophic actinic keratoses and squamous cell carcinoma were treated with cryotherapy and excision, respectively. Hydroxyurea induces a variety of cutaneous side effects such as painful leg ulcers and squamous cell carcinomas. Given the wide variety of adverse cutaneous side effects associated with long-term hydroxyurea therapy, the first step in management is to insure that physicians and patients are aware of the specific risks of this treatment. Patients under hydroxyurea therapy should be monitored closely by dermatologists to early detect and treat the cutaneous side effects.
