Staining methods of the duodenal and intestinal mucosae

Résumé

En Europe, dans les années 1970, à la suite des auteurs japonais, les méthodes de coloration de muqueuse duodénale ont été centrées sur les mini-lésions et l’ulcère duodénal. L’introduction d’antisécrétoires puissants (IPP) et l’éradication de H. pylori ont par la suite, réduit l’intérêt des colorations.

En revanche, les lésions d’atrophie villositaire des muqueuses duodénale et jéjunale sont devenues l’enjeu des colorations vitales et de surface.

La disponibilité récente de la vidéoendoscopie à haute résolution avec optique grossissante et coloration vitale est désormais l’outil performant du diagnostic et du suivi des atrophies villositaires totales ou partielles dans la maladie cœliaque, ses variantes (MC latente, sprue réfractaire) et d’autres syndromes de malabsorption (sprue tropicale, etc.).

Summary

In the seventies in Europe, after the Japanese, duodenal mucosa staining methods were focused on minute lesions and duodenal ulcer. The availability of more powerful acid secretion inhibitors (PPI) and H. pylori eradication have decreased the interest for vital coloration. However, the diagnosis of duodenal and jejunal villous atrophic lesions required vital dyes and spraying coloration techniques. Nowadays, the use of megapixel videoendoscope with magnifying optical system associated to chromoscopy is powerful for the diagnosis and the follow-up of total or subtotal villous atrophis in celiac disease or its variants (latent CD, refractory sprue) and in other malabsorption syndrome diseases (tropical sprue), etc.
