Gebruik en dosering van aminofyllinezetpillen

Abstract

The necessity of an individual theophylline dosage regimen — which is independent of the route of administration — is discussed. In view of the large interpatient variability in clearance rate clinical assessment of the optimal dosage regimen is desirable, especially in acute situations. The applicability of aminophylline suppositories is also reviewed. From pharmacokinetic data it can be deduced that such suppositories are no appropriate dosage forms for acute pharmacotherapy and do not deserve a prominent place as far as chronic pharmacotherapy is concerned. As a consequence, the high frequency of prescribing aminophylline suppositories, as occurs now in The Netherlands, is questionable.

Samenvatting

Er wordt ingegaan op de noodzaak om theofylline, onafhankelijk van de toedieningsweg, individueel te doseren. Gezien de grote interindividuele variatie in klaringssnelheid is een klinische instelling, met name in acute situaties, wenselijk. Vervolgens wordt aandacht besteed aan de toepassingsmogelijkheden van aminofyllinezetpillen. Uit farmacokinetische gegevens valt af te leiden dat deze niet geschikt zijn voor acute farmacotherapie en geen vooraanstaande plaats verdienen wat chronische farmacotherapie aangaat. Bijgevolg dient er een vraagteken te worden geplaatst bij het grote verbruik van aminofyllinezetpillen in Nederland.
