Changes in middle ear pressure following operations in the epipharynx

Summary

The influence of tonsilleetomy and adenotomy on tube function was examined in 100 children ranging from 2 years to 12 years of age.

The measurements were performed by tympanometry during 6 days after the operation. The results were put into relation to the pressure values found before operation.

In cases with normal middle ear pressure values these declined on the first day after operation down to values below 150 mm (watergauge) normalizing within the following 5 days. In cases with preoperative fluid in the middle ear the tympanogram would mostly not normalize after adenotomy.

The results of these examinations give rise to consequences to the therapeutic procedure.

    The adenotomy never should be performed together with tympanoplastic surgery. Otherwise the subpressure values after adenotomy may lead to adhesive processes.

    As middle ear fluid will not be absorbed after adenotomy alone tubes should be inserted into the tympanic membrane.
