The operative findings in anterior cranial fractures in relation to laceration of the dura mater
During the period 1970–1974, 31 cases were diagnosed at operation to have a laceration of the dura mater of the anterior cranial fossa. In only 7 of these was there a diagnosis of laceration made preoperatively—4 cases had cerebrospinal fluid rhinorrhoea and 3 had a pneumatocele demonstrated radiologically. All 31 patients sustained injuries of recent origin and were operated on within 7 days.
The difficulty in diagnosis of recent dural lacerations is discussed. The importance of operating on cases demonstrating the suspicion of a fracture of the anterior cranial fossa or those demonstrating a simple fracture of the sinus is mentioned.
The discrepancy between the preoperative and the operative findings of a durai laceration of the anterior cranial fossa is discussed.
