In the original publication of the article, oxycodone dose was missing from the column heading of Tables 1 and 2, classification column was inadvertently added to Table 1, and the number of beds for each hospital were presented as absolute numbers instead of a range in Table 1. The correct Tables 1 and 2 are published here.
